Rally Jamaica Entry Form
Please complete fully and return to the Jamaica Millennium Motoring Club (JMMC) office via fax at: 876-934-1288 or email to: jamotoringclub@gmail.com by November 11, 2011 . International competitors can send in applications via the Overseas Liaison Officers, Michael Fennell Jr. at mikef@cwjamaica.com or Marcia Dawes-Lamey at rallyprincess@gmail.com, or as per other instructions from your respective country liason.
ALL signed entries MUST be submitted in ORIGINAL form to the JMMC office. Fax copies are accepted only provisionally.
